煤炭总医院门诊合理用药国际指标调查 Meitan General Hospital Investigation of International Indicators of Rational Drug Use in Outpatient 单文卫 煤炭总医院 北京 （100028） Shanwenwei Meitan General Hospital ,Beijing, China 100028 Abstract：Objective To understand the situation of rational drug use in our hospital，to analyze their reasons and to study the measures for raising the level of promoting rational drug use．Methods According to the rational drug use indicators recommended by WHO，l 000 outpatient prescriptions were randomly selected for spot check during 1~5 months and 200 outpatients within l month were investigated．Results In the prescription indicators，the average number of drug varieties per prescription was 3.83，the usage rate of antimicrobial drugs was 58.7％，the rate of injection use was 33.1％ and the usage rate of basic drugs was 87.6％；in the patients care indicators，the average see—doctor time was 15.6 min，the average dispensing time was 22.1s，the actual dispensing rate of drugs wag 97.3％，the integrity rate of drug labeling was 100％，the rate of patients understanding the correct usage of drugs Wag 74.1％．Conclusion The outpatient prescription indicators are still in the high side．The usage rate of basic drugs and the patient care indicators are relatively in lower level．The level of rational drug use needs to be improved．Doctors and pharmacists should further strengthen to exchange with patients and take care of patients． Keywords: drug use；international indicators 摘要：研究我院临床合理用药状况，促进合理用药水平的提高。方法：选择选择我院2011年1~5月门诊处方和当日门诊就医患者，采取多中心、随机、平行试验方法，时合理用药国际指标的核心指标进行现场调查，并进行统计、分析。结果：相关处方指标平均用药品种数3.8种、抗生素应用率58.7％、注射液应用率33.1％；相关患者关怀指标平均就诊时间15.6分钟、平均调配药时间22.1秒、实际调配药物率97.3％、药物标示完整率100％、患者了解正确用药方法率74.1％。结论：煤炭总医院合理用药水平目前还存在较多问题，医师、药师对患者的交流关怀不够，有待提高和进行干预。 关键词：合理用药；国际指标；现场调研 目前造成患者医疗负担的主要原因是药品的不合理使用。药品的不合理使用不仅是阻碍临床医疗质量提高的重要因素，也是造成医药资源浪费的主要原因。1985年，世界卫生组织(WHO)提出了实施基本药物政策和推动临床合理用药，［1］并制定了合理用药国际指标（selected drug use indicators，SDUIs）包括药物的利用信息，医师、药师服务的人文指标，还有补充指标和行政管理指标，以便能较客观、科学地评价不同国家和地区对不同条件的医疗机构临床
