This modality has enabled surgeons to minimize operative blood loss and decrease operative time, thus positively affecting patient morbidity and mortality. Electrosurgery, however, remains one of the most hazardous surgical modalities available in the surgeons armamentarium. Yet, relatively few surgeons or nurses have a sufficient understanding of the principles of electrosurgery. Consequently, they do not always know why or how a patient was injured during the use of electrosurgery. Injuries rarely occur because of a generator malfunction. Rather, they occur because of inappropriate use of the PRE (the dispersive pad), active electrode (the pencil), and electrosurgery accessories. This program provides an overview on the safe use of patient return electrodes. The patient’s skin condition should be assessed and documented before and after ESU use. Single use patient return electrodes should be an appropriate size for the patient and not altered (cut or folded). Using a large dispersive electrode prevents concentration of current and minimizes the potential for electrosurgical injuries. The AORN recommended practices continue advising that personnel verify that the electrode is intact and if there is conductive gel, that it be moist and that the manufacturer’s expiration date has not been reached. Single use patient return electrodes should maintain uniform body contact. Potential problems include tenting, gaping, and moisture that interfere with adhesion to the patient’s skin. Dispersive pads should be kept dry and protected from seeping or pooling of fluids. Liquids may prevent the electrode from adequate contact to skin. AORN recommends using the manufacturer’s written instruction for safe use and that those instructions should be on the packaging as well as the box.
