肘关节滑膜骨软骨瘤病的影像诊断分析 程小杰1，姚庆东1，柳海晓1，何家维1，张呈兵1，彭磊*1，2 (1.温州医学院附属第二医院；浙江 温州 325000；2.海南医学院附属医院,海南 海口 570100） 【摘要】 目的 探讨肘关节滑膜骨软骨瘤病的影像学特征。 方法 回顾性分析14例经手术病理证实的肘关节滑膜骨软骨瘤病患者的临床表现和X线、CT和MRI资料。结果 本组14例中，男10例，女4例，平均年龄41.9岁。单肘关节发病13例，较少见的双肘发病1例，右肘12例，左肘3例。9例在X线平片上均表现关节内为多个圆形或卵圆形的钙化结节影，大小、数目不等，结节密度不均，呈环形或半环形高密度影。6例多层螺旋CT薄层扫描清晰显示大小不等、数目不一的钙化游离体，可显示X线阴性的隐蔽部位游离体、增厚的滑膜、软骨下侵蚀和关节腔内积液情况。1例MR检查发现肘关节内多个结节，边界清晰，在T1WI呈等、低信号，在T2WI和STIR序列呈低信号，在T2WI脂肪抑制序列上显示滑膜增厚，关节腔内少量积液。 结论 肘关节滑膜骨软骨瘤病少见，影像学特征典型，结合病史可明确诊断。 【关键词】 肘关节； 滑膜骨软骨瘤病； 放射摄影术； 体层摄影术； X线计算机； 磁共振成像 Imaging diagnosis analysis of elbow joint synovial osteochondromatosis Cheng Xiao-jie1, Yao Qing-dong1, Liu Hai-xiao, He Jia-wei1, Zhang Cheng-bing1, Peng Lei*1,2 (1.The Second Affiliated Hospital of Wenzhou Medical College, Wenzhou 325000, China; 2. Methods 14 cases elbow joint synovial osteochondromatosis proved by surgery and were collected in this study. The clinical manifestation and imagings finding X-ray, CT, MRI were analyzed retrospectively. Results Of 14 cases, 10 males, 4 females，age is 41.9 years. Unilateral elbow joint involved in 13 cases, 1 case involved bilateral elbow joints rarely, the right elbow joint involved in 12 cases, 3 cases in the left. Multiple intra-articular calcification or nodules in round or oval could be found in all 9 cases on radiograph, the nodules appeared unequally in magnitude and amounts, uneven density, and presented ring or half ring of high-density. 6 cases executed by MSCT thin slice scan, calcification displayed sizes, amounts in variety, furthermore, loose bodies in concealed site which were negative on X-ray could be demonstrated on MSCT as well as thickened synovial, cartilage erosion and the articular cavity effusion.
