INSTRUCTIONS FOR COMPLETING THE UNIVERSITY OF TEXAS ANIMAL UTILIZATION PROPOSAL FORM (AUP) The Animal Welfare Act and the PHS Policy on Humane Care and Use of Laboratory Animals require review and approval of an Animal Research Proposal by the appropriate Institutional Animal Care and Use Committee (IACUC) prior to the conduct of any activity involving vertebrate animals. If this is a continuation, modification, or renewal you only need to use the appropriate form letter listed as attachments 1 and 2. This should also include the UT cover page with budget information and a copy of the summary or abstract page. When completing the AUP make sure you have included all information needed, including the correct number of copies, one for OSP, one copy for the PI and fifteen for the IACUC review. Also include a copy of the UT cover page with the budget information and the summary or abstract page. It should be noted that the requirements for verification of IACUC approval as part of the application process are found in PHS Policy (IV. D. 1 2), the PHS Form 398 instructions, and the NIH Grants Policy Statement. They are based on statutory requirements of the PHS Act. Although institutions are allowed maximum flexibility within this Policy (e.g., by not prescribing IACUC responsibility for side-by-side review), the requirements for congruency between the animals activities described in grant applications and IACUC approvals remain unchanged. Institutional failure to comply with these Policies (terms and conditions of award) may result in the deferral of grant applications, loss of PHS support, and/or restriction or withdrawal of the OLAW (Office of Laboratory Animal Welfare)-approved institutional Animal Welfare Assurance. In “performance standard” terms, the outcome must be appropriate correlation between application and protocol, regardless of the specific mechanism chosen by the institution to achieve it.
