肺部孤立性结节或肿块MSCT灌注成像与微血管密度的相关性研究 刘卫金,张冬,杨春艳,邹利光,廖翠薇,王文献,戴书华 (第三军医大学新桥医院:放射科,重庆 400037) 摘 要 目的 探讨肺部孤立性结节或肿块MSCT灌注成像与微血管密度(microvessel density,MVD)的相关性。方法 对33例肺部孤立性结节或肿块患者于经皮肺穿刺活检或手术前行16层螺旋CT灌注扫描,病理标本常规HE染色及单克隆抗CD34抗体标记血管,将灌注参数值和微血管密度进行对照分析。结果 周围型肺癌(恶性组)20例,活动性炎性结节或肿块(炎性组)5例,其他良性结节或肿块(良性组)8例。恶性和炎性组的MVD明显大于良性组(P0.01),而恶性与炎性两组间MVD没有统计学差异(P0.05)。除MTT外,BF、BV和PS与MVD之间均呈线性正相关(P0.01),其中以BV值相关系数最高。结论 肺结节或肿块的BF、BV和PS值与MVD之间呈正相关,MSCT灌注成像为在活体无创性评价肺肿瘤的微血管密度情况提供了一条新的途径。 关键词:孤立性结节;肿块;肺;微血管密度;灌注成像;体层摄影术,X 线计算机 Correlative study between outcomes perfusion parameters of multi-slice CT and microvessel density of lung solitary nodule or mass LIU Wei-jin,ZHANG Dong,YANG Chun-yan,ZOU Li-guang,LIAO Cui-wei,WANG Wen-xian,DAI Shu-hua ( Department of Radiology,Xinqiao Hospital,Third Military Medical University,Chongqing 400037 ) Abstract Objective To evaluate the relationship between outcomes perfusion parameters of multi-slice spiral CT(MSCT) and microvessel density(MVD)of lung solitary nodule or mass. Methods A total of 33 patients with lung solitary nodule or mass performed MSCT perfusion scanning with GE Light-Speed ultra 16 CT scanner before CT-guided percutaneous pulmonary biopsy or operation, and the HE and immunohistochemistry staining of the specimen were acquired. CT perfusion parameters and MVD of the 33 cases were analyzed comparatively. Results Among all 33 cases, there were 20 cases of peripheral lung cancer, 5 cases of acute inflammation and 8 cases of the other benign solitary nodule or mass. MVD of malignant and inflammatory cases were significantly higher than that of benign ones(P0.01), while no statistically significant difference was found between malignant and inflammatory cases(P0.05). BF, BV and PS were correlated with MVD respectively (all P0.01), but MTT were not. Conclusion BF, BV and PS were correlated with MVD respectively in lung solitary nodule or mass, MSCT perfusion imaging provides an effective method to assess MVD of lung cancer in vivo. Key words: s
