中年女性动态心电图一过性ST段改变的临床分析30例 　　doi：10.3969/j.issn.1007-614x.2014.1.56 　　摘 要 目的：通过动态心电图ST段改变与冠状动脉造影结果对照，探讨中年女性动态心电图ST段改变在冠心病心肌缺血的诊断价值。方法：入选30例年龄30～55岁，平均42.5岁的女性患者。超声心动图、血生化检查阴性，动态心电图ST段改变符合冠心病“三个一”诊断标准。排除束支阻滞、预激综合征、高血压、高血糖、洋地黄作用，起搏心律、超声心动图排除严重瓣膜或先天性心脏病、心室肥大等器质性改变，与冠状动脉造影对照。结果：45～55岁10例患者中，冠状动脉造影阳性3例（10%）。结论：对中青年女性患者动态心电图一过性ST段改变达到诊断冠心病一过性心肌缺血的诊断标准，不能盲目诊断为冠心病，但也不能疏忽，需充分结合临床及其他病史资料进行综合性分析而作出诊断。 　　关键词 ST段改变 冠状动脉造影 冠心病 女性 　　Abstract Objective：Through the control of dynamic electrocardiogram ST segment changes and the results of coronary angiography of middle-aged women，dynamic ECG ST segment changes in diagnosis of myocardial ischemia in patients with coronary heart disease.Methods：30 patients aged 30 to 55 years with an average of 42.5 years old female patient.Ultrasonic heartbeat graph，negative blood biochemical examination，dynamic ST segment of ECG changes consistent with coronary heart disease “three in one” diagnostic criteria.Exclusion of bundle branch block，pre excitation syndrome，hypertension，hyperglycemia，digitalis effect，pacing rhythm，ultrasound heartbeat graph without severe valvular or congenital heart disease，ventricular hypertrophy and other organic change，compared with coronary artery angiography.Results：in 45～55 years old in 10 patients，coronary angiography were positive in 3 cases，accounting for 10%.Conclusion：the young women of dynamic electrocardiogram in patients with transient ST segment changes to the diagnosis of coronary heart disease diagnostic criteria of transient myocardial ischemia，not blindly for the diagnosis of coronary heart disease，but also can not be ignored，should be combined with clinical and other medical history data comprehensive analysis and diagnostic. 　　Key word ST segment changes；Coronary angiography；Coronary heart disease；Women 　　动态心电图中一过性ST段改变是临床诊断冠心病心肌缺血的重要依据，但这种改变受多种因素影响，本文通过对30例体检阴性、心脏无明显器质性病变的中年女性一过性ST段改变进行回顾性分析、总结经验，指导临床共同提高对这种改变的认识。 　　资料与方法 　　一般资料：2012年1月-2013年1月收治疑似冠心病女性患者30例，年龄30～55周岁。 　　方法：①所有入选病例均进行静息心电图、
