纳洛酮治疗肺性脑病38例临床分析 　　【摘要】目的观察纳洛酮在治疗Ⅱ型呼吸衰竭并发肺性脑病的临床疗效。方法将76例诊断明确的Ⅱ型呼吸衰竭肺性脑病，随机分为治疗组、对照组，两组均给抗感染，持续低流量给氧，保持呼吸道通畅，维持水电解质及酸碱平稳，营氧支持，对症治疗原发疾病等综合措施。治疗组在此基础上加用纳洛酮注射剂，首次剂量为08 mg静脉推注，以后给纳洛酮2 mg静脉持续滴注，2次/d，连续三天。结果动脉血气分析指标：治疗组治疗前后动脉血气分析值比较P001；对照组总有效率为701%，治疗组总有效率914%，治疗组显著高于对照组（P001）。结论纳洛酮治疗Ⅱ型呼吸衰竭肺性脑病安全性较高、临床疗效显著，值得临床推广。 　　【关键词】 　　纳洛酮；Ⅱ型呼吸衰竭肺性脑病 　　Clinical analysis of pulmonary encephalopathy treated by Naloxone in 38 cases 　　WANG Andong Department of ICU Shangqiu First Peoples Hospital， Henan Province， 476100， China 　　【Abstract】Objective 　　To observe the clinical effects of naloxone in the treatment of type II respiratory failure with pulmonary encephalopathy Methods76 patients diagnosed with type II respiratory failure and pulmonary encephalopathy， were randomly divided into treatment group and control group， two groups were given antiinfection， persistent low flow oxygen， maintain airway patency， maintain water electrolyte and acidbase stability， camp oxygen support， symptomatic treatment of primary disease and other comprehensive measures The treatment group based on the use of naloxone injection， the first dose of intravenous injection of 08 mg， 2 mg after administration of naloxone intravenous infusion， twice a day， for three consecutive days ResultsThe of arterial blood gas analysis： the treatment group before and after treatment analysis of arterial blood gas values compared to P001； the control group the total effective rate was 701%， the total effective rate of treatment group 914%， treatment group was significantly higher than that of the control group （P001） ConclusionThe curative effect of naloxone in the treatment of type II respiratory failure and pulmonary encephalopathy higher security， obviously， is worth the clinical promotion. 　　【Key words】 　　Naloxone； Type II respiratory failure and pulmonary encephalopathy 　　作者单位：476100河南省商丘市第一人民医院重症监护科 　　临床上肺性脑病是指由于慢性胸肺疾病导致患者出现高碳酸血症、严重低氧血症进而继发的一系列中枢神经系统功能障碍综合征。在治疗上主要是常规给予患者抗生素抗感染，维持酸碱平衡，及时纠正水电解质紊乱，改善患者呼吸功能，
