碱性尿路感染中解脲棒杆菌培养的分析 【摘要】 目的：回顾性调查引起碱性尿路感染的病原菌中解脲棒杆菌的发生率并分析工作中注重此菌培养的重要意义。方法：对本院三个重要科室住院患者的清洁中段尿培养阳性且尿常规分析显示为碱性尿路感染及(或)伴有尿液结晶的病例资料进行回顾性调查，并对收集到的6株解脲棒杆菌重新进行鉴定和药敏分忻。结果：在入选合格的245份病例中，解脲棒杆菌的分离，阳性的仅为6份，但在这6份病例中，解脲棒杆菌培养阳性对临床诊治起到重要的指导作用。解脲棒杆菌多重耐药，仅对万古霉素、替考拉宁及左氧氟沙星保持较高敏感性。结论：解脲棒杆菌在碱性尿液的尿路感染患者中的阳性分离率很低，但因其培养较困难且多重耐药严重，常导致临床治疗失败，所以提高阳性检出率，尤其是对于难治性、顽固性碱性尿感、尿路结石的患者明确诊疗发挥关键性的作用。 【关键词】 碱性尿感;解脲棒杆菌;尿路结石;多重耐药 　　Analysis of Corynebacterium Urealyticum Culture in Alkaline Urinary Tract lnfection 　　YANG Wei，ZHAN BiCui 　　(Hangzhou Hospital of Traditional Chinese medicine, Hangzhou, Zhejiang 310007, China) 　　Abstract：Objective To investigate the occurrence percent of Corynebacterium urealyticum in pathogen which can cause alkaline urinary tract infection retrospectly and analyze important significance of attention to Corynebacterium urealyticum culture in daily work． Methods In retrospect investigate the inpatient case information from three main ward which show positive urinary culture results and the analysis of urinary routine show alkaline urinary tract infection and(or) in camparison with urinary struvite crystalsin order to find out the pathogen distribution in alkaline urinary tract infection． The identification test and AST of the collected six Corynebacterium urealyticum strains were done again． Results Only six Corynebacterium urealyticum strains were isolated from 245 cases which meet the needed conditions． However， the positive urinary culture results of Corynebacterium urealyticum play important roles in helping the clinical diagnosis and treatment． Corynebacterium urealyticum is multiresistant only sensitive to Vancomycin， Teicoplanin and Levofloxacin． Conclusion Despite the isolated percent is low， the Corynebacterium urealyticum often can lead to the failure of the clinical diagnosis and treatment for its culture difficulty and mutiresistant drug.Therefore,the improvement of positive isolation rate is key to helping the cli
