Methods From June 2008 to March 2012, 25 patients with old posttraumatic wry nose deformity were treated under the general anesthesia. The nasal bone was exposed through the transcartilage incision and corrected by lateral nasal osteotomy and central nasal osteotomy. The nasal septum cartilage was exposed through the columella combined with the alar cartilage. The deviated nasal septum cartilage was corrected by release or excision. Results The period of follow-up ranged from 3 months to 1 year. The outcome was excellent in 21 patients and good in 4 patients. The nasal deformities were corrected satisfactorily with no complication. Conclusion Simultaneous correction of nasal bone and nasal septum cartilage has good effect on old posttraumatic wry nose deformity, and it is worth clinical promotion. [Key words] nasal bone；nasal septum；cartilage；rhinoplasty Corresponding author: Li Shirong, E-mail：zhengxing@ 0.2cm以上的外鼻畸形[1]。我们自2008年6月至2012年3月，对25例陈旧外伤性歪鼻畸形患者同期行鼻骨和鼻中隔软骨矫正，效果良好，现报告如下。 1 临床资料 1.1 一般资料 选取2008年6月至2012年3月在第三军医大学西南医院整形美容外科收治的陈旧外伤性歪鼻畸形患者25例患者，其中男性18例，女性7例；年龄28.6(18~42)岁。所有患者具有3个月以上明确鼻外伤史，均伴鼻中隔偏曲，20例伴通气障碍。术前行鼻部轴位及冠状位CT扫描，明确歪鼻畸形的类型、移位骨折块的空间结构关系及鼻中隔偏曲的情况。歪鼻按形态分为3类[2]：侧斜型8例，“C”型歪曲12例，“S”型歪曲5例。麻醉采用全麻经口腔插管。 1.2 手术方法
