万古霉素微量泵控制持续鞘内注射治疗颅内感染 严建泉，钱东翔，罗 鹏，王智坚 （广州医学院第三附属医院 神经外科，广东 广州，５１０１５０） 摘 要：目的 探讨利用微量泵控制下持续鞘内注射万古霉素治疗颅内相关感染的临床疗效。方法 观察 ２００９ 年 １ 月 —２０１２年 １０ 月本院收治的各类颅内感染病例 ５０ 例，其中 ２２例行腰大池引流脑脊液并分次鞘内注射万古霉素治疗作为对照 组，２８ 例在上述基础上改用微量泵控制下持续鞘内注射万古霉素治疗为实验组，观察 ２组间的疗效差异。结果 实验组对体 温、血象、脑脊液等的观察指标较对照组改善迅速，而腰腿痛、抽搐、头痛等副反应观察指标较对照组明显减少。结论 腰大 池持续引流脑脊液并微量泵控制下持续鞘内注射万古霉素是治疗颅内感染的一种安全有效的方法。 关键词：万古霉素；颅内感染；鞘内注射 中图分类号：R ６５１．１ 文献标志码：A 文章编号：１６７２-２３５３（２０１４）０７-０９１-０３ DOI：１０．７６１９ ／jcmp．２０１４０７０３０ Microinjection pump control continuous intrathecal injection of vancomycin for the treatment of intracranial infection YAN Jianquan，QIAN Dongxiang，LUO Peng，Wang Zhijian （Department of Neurosurgery，The Third Affiliated Hospital of Guangzhou Medical College， Guangzhou，Guangdong，５１０１５０） ABSTRACT：Objective To study the clinical curative effect of continuous intrathecal injec- tion of vancomycin controlled by microinjection pump in the treatment of intracranial infection． Methods ５０ patients with intracranial infections from January ２００９ to October ２０１２ were includ- ed in the research．２２ patients treated with cerebrospinal fluid drainage from lumbar cistern and discontinuous intrathecal injection of vancomycin were in control group，and another ２８ patients with continuous intrathecal injection of vancomycin by micropump were in the experimental group． Therapeutic effect was observed between the two groups．Results The patients in the control group improved more rapidly than those in the experimental group in the temperature，blood，cere- brospinal fluid and other observation index．Compared with the control group，the pain of lower back and leg，convulsions，headache and other side effects were significantly reduced in the experi- mental group．Conclusions Continuous drainage of cerebrospinal fluid in lumbar cistern and mi- croinjection pump control continuous intrathecal injection of vancomycin is a safe and effective method to treat intracranial infection． KEY WORDS：vancomycin；intracranial infection；intrathecal injection 颅内感染是一类严重的感染性疾病，广
