·临床研究· 2013年12月第3卷第23期 克氏针卡压固定治疗指骨末节基底背侧撕脱骨折 刘晓宾? ? 谷右天 山东省聊城市中医医院，山东聊城 252000 [ 摘要 ] 目的 介绍一种应用克氏针卡压固定末节指骨背侧撕脱骨折的手术方法。 方法 对 14 例 14 指末节指骨基 底背侧撕脱骨折，采用切开复位，末节指骨过伸固定远指关节，克氏针斜形卡压撕脱骨块的方法，术后撕脱骨块均 达解剖复位，6 周拔除克氏针作主被动功能锻炼。 结果 术后随访 3~6 个月经 TAM 评定：优 11 例，良 3 例。 结论 骨折全部早期愈合，采用克氏针卡压固定末节基底背侧骨折，能有效治疗骨性锤状指畸形，具有手术操作简单、创 伤小、疗效满意等优点。 [ 关键词 ] 骨折；外科手术治疗；克氏针固定 [ 中图分类号 ] R683.41? ? ? Youtian Liaocheng City Hospital of Traditional Chinese Medicine, Liaocheng 252000, China [Abstract] Objective To introduce one surgical method of applying Krischner wires fixation in the basal and dorsal avulsion fracture in phalangette. Methods Krischner wires were put obliquely in 14 cases of patients with basal and dorsal avulsion facture in phalangette to restore the fracture after open reduction and making the phalangette pass thourgh distal phalangeal joint. All the cases were successful and removed the Krischner wires in week six. Then they received functional rehabilitaion. Results All the patients were followed up for 3 to 6 months and the TAM evaluation showed excellent results in 11 cases and good results in 3 cases. Conclusion Krischner wire fixation in the basal and dorsal avulsion fracture in phalangette is very effective in the treatment of bony mallet finger with the advantages of simplicity of manipulation， micro-invasion， and satisfactory effect.
