·1 1 · 电子胎 心监护诊 断胎儿窘迫 的临床观察 丁秀萍 [ 摘要】 目的 探讨 电子胎心监护与胎儿窘迫及剖宫产率之间的关系。方法 回顾分析4 647 例 住院分娩孕妇的胎心电子监护 图形 。结果 发现胎心监护图形异常者38 2 例 ，其中223 例考虑胎儿窘迫 行剖宫产术 ，术 中发现异常因素者 71 5 例 ，未发现异常因素者48 例 。胎心基线变异明显减弱或消失 、重 度变异减速 、延长减速及晚期减速者发生羊水粪染 、脐带缠绕及新生儿窒息比例 明显高于其它胎心监护 图形异常者 结论 电子胎心监护能早期发现胎儿窘迫 ，但是 ，单凭胎心监护 图形异常作为胎儿窘迫诊 断会 出现假 阳性判断 ，当出现异常图型时，应严密监护 ，根据胎心率异常的程度及胎儿能够娩 出的时间 选择恰 当的分娩方式 ，可减少 围产儿病死率 。 【关键词 】 电子胎心监护 ;胎儿窘迫 ;剖宫产率 The Clin ical 0 b ser vat ion of E lectr onic F e扭l Hear t Rate on ito r iug in Dia gn osing Fet al Distr ess DNI C Xiu一ing . Conclusion Electro nic ef tal heart rate monitori ng could if nd ef tal distress in time ，but if we diagnose ef tal distress only rely on the electro nic ef tal heart ra te monitoring ，we would draw a af lse positive j udgmenl ，when the abnormal Pattern s aPPear ，we should take a close observ ation ，according to ht e ba normal de- gre e and the given laboring time we could choose a corr ect labori ng method to re duce the death rate of newborn .
