我中心奥氮平致不良反应报告分析 ＊ 马 燕 ，李方捷，毛叶萌（上海交通大学医学院附属精神卫生中心药剂科，上海 200030） + 中图分类号 R971.4；R969.3 文献标志码 C 文章编号 1001-0408（2013）24-2235-03 DOI 10.6039/j.issn. 1001-0408.2013.24.08 摘 要 目的：探讨奥氮平致不良反应（ADR ）的特点和规律，为临床合理用药提供参考。方法：对2006年1月－2012年10月收 集上报的189例奥氮平致ADR 进行回顾性调查、分析。结果：奥氮平最常见的ADR 为中枢及外周神经系统损害，共67例（占 32.21％），主要临床表现为锥体外系反应、嗜睡、头晕等；其次为肝胆系统损害，共27例（占12.98％），表现为肝功能异常、丙氨酸氨 基转移酶升高。ADR 发生在奥氮平用药1周内的有46例（占24.34％），2个月内的有145例（占76.72％）；剂量在12.5～20mg/d 范 围内引起的ADR 最多，共117例（占61.90％）。ADR 经治疗后147例（占77.78％）好转，38例（占20.11％）治愈，4例（占2.12％）有 后遗症。结论：尽管奥氮平相比典型抗精神病药物安全性和耐受性较好，但其引起的ADR 仍需要高度重视，加强监测。 关键词 奥氮平；药品不良反应；分析 Analysis of Adverse Drug Reaction Induced by Olanzapine in Our Center MA Yan ，LI Fang-jie ，MAO Ye-meng （Dept. of Pharmacy ，Mental Health Center Affiliated to Shanghai Jiaotong University School of Medicine ，Shanghai 200030，China ） ABSTRACT OBJECTIVE ：To investigate the characteristics and regularity of adverse drug reaction （ADR ）induced by olanzap- ine ，and to provide reference for rational drug use in the clinic. METHODS ：ADR induced by olanzapine reported by our center from Jan. 2006 to Oct. 2012 were investigated and analyzed retrospectively. RESULTS ：The most common ADR induced by olan- zapine were central and peripheral nervous system disorders （67 cases ，32.21％），manifesting as extrapyramidal side effects ， drowsiness ，dizziness ，etc. The second were liver and biliary system disorders （27 cases ，12.98％），including abnormal liver func- tion ，elevated aminotransgerase ，etc. There were 46 ADR cases within a week （24.34％），and 145ADR cases within 2 months （76.72％）. 117ADR cases occurred in olanzapine doses of 12.5-20 mg/day ，accounting for 61.90％. After treatment ，147 cases were recovered （77.78％），38cases were cured （20.11％），and 4 cases had sequela.
