择期PCI围手术期心肌损伤中西医探究进展 　　摘要：冠状动脉粥样硬化性心脏病（CHD）又称为缺血性心脏病，包括冠状动脉粥样硬化使血管腔狭窄或阻塞的器质性病变和冠状动脉痉挛的功能性改变所引起的以心肌缺血缺氧和坏死为表现的心脏病，属临床常见病、危重病。经皮冠状动脉介入治疗（percutaneous coronary intervention，PCI）技术是解决血管狭窄病变的一个重要方法，广泛应用于冠心病患者，对改善患者预后有明显积极效果。随着设备的不断更新和技术的不断提高，围手术期心肾损伤发生率虽较前降低，但是仍存在很大的比率，特别是择期PCI围手术期心肌损伤（Peri-procedure Myocardial Injury，PMI）受到越来越多的关注。现就中西医结合防治择期PCI围手术期心肌损伤进行探讨 关键词：择期PCI围手术期；心肌损伤；中西医结合；研究进展 Abstract：Coronary heart disease（CHD） is also known as ischemic heart disease，including coronary functional changes in vascular stenosis or obstruction of the organic disease and coronary artery spasm caused by hypoxia and necrosis of myocardial ischemia as a manifestation of heart disease is a common clinical disease in critically ill patients.Percutaneous coronary artery interventional treatment（percutaneous coronary intervention PCI） technology is an important method to solve the vascular stenosis，widely used in patients with coronary heart disease，have obvious positive effect on improving the prognosis of patients.With the constantly updated equipment and technology to improve the peri operative incidence of heart kidney injury is lower than before，but there are still a lot of ratios，especially elective PCI perioperative myocardial injury（Peri-procedure Myocardial，Injury， PMI） has attracted more and more attention.The combination of traditional Chinese and Western medicine treatment of elective PCI perioperative myocardial injury operation period were discussed. Key words：Elective PCI perioperative period；Myocardial injury；Integrated traditional Chinese and Western medicine；Research progress 随着冠心病患者的增多，PCI作为血运重建治疗闭塞性冠状动脉疾病的重要方法，广泛应用于重度冠状动脉狭窄的患者。研究发现[1]，围术期心肌损伤（PMI）在冠心病PCI?g患者中发生率约33%。因此，择期PCI围手术期心肌损伤的防治是一项重要课题。而现代医学虽发挥了主导作用，但仍存在一定的局限性，研究发现中医中药对防治围手术期心肌损伤具有一定的积极作用，因此探讨围手术期心肌损伤机制，充分发挥传统医学与现代医学各自优势，中西医结合防治围手术期心肌损伤尤为必要 1 围手术期心肌损伤定义 在2012年的第三版《全球心肌梗死新定义》[2]中，PMI定义为肌钙蛋白（cTn）在PCI术后48 h内的异常增高。对于术前正常的肌钙蛋白（cTn），术后48 h内明显升高5倍99%正常参考值上限，统称为PCI围手术期心肌损伤。同时应包括下列证据之一：①有心肌缺血症状；②心电图检查提示
