小儿盘状半月板的MRI诊断与分型探讨 黄显龙，杨华，陈修燕，李必强，王凡，王新玉（重庆市第三人民医院放射科，重庆） 摘 要: 目的 探讨小儿盘状半月板的MRI表现与分型。方法 分析37例（42膝）小儿盘状半月板的MRI表现，初步总结MRI诊断与分型要点。 结果 37例（42膝）中22膝半月板前角、体部、后角均增厚，呈板状增厚改变 ，相应关节软骨较对侧变薄；15膝半月板前角或后角肥厚，对应关节软骨变薄，半月板体部改变不明显，呈肥角状改变；5膝半月板前后角均增厚，体部略增厚呈领结状改变。根据半月板形态的改变将其归为3种类型：即板型、肥角型、领结型。 结论：MRI能准确显示盘状半月板的病变范围 和程度以及合并的半月板损伤；对盘状半月板具有定性诊断及分型价值，从而为临床确定治疗盘状半月板的方案提供依据。 关键词: 小儿 ； 盘状半月板； MRI ；诊断 ； 分型 Preliminary Investigation of the MRI Diagnosis and Classification of Young Children’s Discoid Meniscus HUANG Xian-long ,YANG hua ,CHEN Xiu-yan，LI Bi-qiang , WANG Fan,WANG Xin-yu(Department of Radiology, the Third People’s Hospital of Chongqing, Chongqing 400014, China) Abstract: Objective To explore the MRI Findings and Classification of Discoid Meniscus of Young Children’s Discoid Meniscus. Methods To obtain an intial conclusion about key points of the MRI Diagnosis and Classification of Young Children’s Discoid Meniscus,the findings of 37 cases (42 knee joints involved) with proved Young Children’s Discoid Meniscus were analyzed. Results Among 37 cases (42 knee joints involved) ,the thickness of anterior horn,midbody,posterior horn of menisci in 22 knee joints was greater than those of normal menisci ,with the thickness of their cartilage less than those of the opposite,which was characterised by slabby thickening.The thickness of anterior horn or posterior horn of menisci in 15 knee joints,with the thickness of their cartilage in a decline,was greater than those of the normal menisci that was featured by an appearance of gross horn,while its midbody didn’t display any apparent change.In 5 menisci of knee joints,”neckite” finding was contributed to the thickening of anterior and posterior horns as well as the slight thickening of midbody.According to the alteration of shape of the meniscus,it can be classified as 3 types,including slab tpye,gross horn type and “necktie” type.
