3.0T磁共振SWI对颅内海绵状血管瘤合并静脉血管瘤的诊断价值 曹代荣,熊美连,邢振,刘颖,王树 【摘要】 目的 探讨磁敏感加权成像(SWI)对颅内海绵状血管瘤(CA)合并静脉血管瘤(VA)的诊断价值。 资料与方法 回顾性分析17例颅内CA合并VA患者的SWI表现,并与常规MRI(共15例)及DWI(共13例)进行对照。 结果 15例患者中,SWI检出CA病灶数33个,VA的引流静脉20条,常规MRI检出CA 20个,引流静脉10条;13例同时行DWI扫描的患者,SWI检出CA 29个,引流静脉16条,常规MRI检出CA 18个,引流静脉8条,DWI检出CA 22个,引流静脉11条。结论 SWI对颅内海绵状血管瘤合并静脉血管瘤的诊断具有重要的临床应用价值。 【关键词】 常规磁共振成像 磁敏感加权成像 海绵状血管瘤 静脉血管瘤 弥散加权成像 The Value of 3.0T MR Susceptibility Weighted Imaging for diagnosing Cavernous Angioma associated with Venous Angioma CAO Dairong,XIONG Meilian,XING Zhen,LIU Ying Department of Images,the First Affiliated Hospital,Fujian Medical University,Fuzhou,Fujian Province 350005,P.R.China 【Abstract】 Objective To investigate the diagnostic value of susceptibility weighted imaging in cavernous angioma(CA)associated with venous angioma(VA).Materials and Methods The SWI datas of 17 patients of CA associated with VA were analysed retrospectively,which were compared to conventional magnetic resonance imaging(15 patients) and diffusion weighted imaging(13 patients). Results In 15 cases,33 carvenous angioma lesions and 20 drain veins of VA were detected by SWI, 20 carvenous angioma lesions and 10 drain veins were deteced on conventional MR sequences.13 cases underwent DWI simultaneously, 29 carvenous angioma lesions and 16 drain veins were detected by SWI, 18 carvenous angioma lesions and 8 drain veins were deteced on conventional MR sequences, 22 carvenous angioma lesions and 11 drain veins were detected by DWI. Conclusion SWI has important clinical value for diagnosing the CA associated with VA. 【Key words】 Conventional magnetic resonance imaging Susceptibility weighted imaging Cavernous angioma Venous angioma Diffusion weighted imaging 颅内海绵状血管畸形(cerebral cavernous malformations,CCM)最早于1854年由luschka描述,其病理基础是由海绵状血管腔隙组成动脉成分的血管畸形SWI是依靠梯度场的切换产生回波回波序列,对主磁场不均匀造成的失相位不但不能剔除,反而暂时性加速了质子的失相位,能反映局部病灶微观磁场的变化对显示出血尤为敏感更易检测出CT常规MR忽略的微小出血或静止性出血,明显提高了CA的检出率。静脉内含有丰富的去氧血红蛋白,表现为顺磁性,能够改变局部磁场,引起周围空间失相位,缩短T2*的驰豫时间,导致信号缺失,是静脉系统的天
