DHS及PFN内固定治疗股骨转子间骨折临床效果及并发症比较[摘要] 目的 对比分析动力髋螺钉（DHS）固定与股骨近端髓内钉（PFN）内固定治疗股骨转子间骨折的临床效果及并发症情况。方法 将我科2005年3月～2010年6月收治的股骨转子间骨折患者94例随机分为DHS组和PFN组，每组47例，应用统计学方法对两组的临床治疗效果及并发症情况进行比较分析。结果 与DHS组比较，PFN组的有效率明显高于DHS组，手术时间及术中出血量明显少于DHS组，骨折愈合时间明显短于DHS组，并发症发生率明显低于DHS组，差异均有统计学意义（P＜0.05）。结论 DHS与PFN内固定治疗股骨转子间骨折均具有较好的疗效，但PFN的并发症少于DHS，因此临床医生需根据手术适应证来确定内固定方法。 [关键词] 股骨转子间骨折；动力髋螺钉；股骨近端髓内钉 [中图分类号] R687.3 [文献标识码] B [文章编号] 1673-9701（2011）26-35-02 Comparisons of Clinical Effects and Complications of DHS and PFN Fixation of Intertrochanteric Fractures ZHOU Hancheng Department of Orthopedics， Hexian Memorial Hospital of Panyu District in Guangzhou City， Guangzhou 511400， China [Abstract] Objective To analyze the clinical efficacy and complications of DHS fixation and PFN fixation of intertrochanteric fractures. Methods Ninty four cases of patients with intertrochanteric fractures treated in our department from March 2005 to June 2010 were randomly divided into two groups： DHS group and PFN group with 47 cases in each； The clinical efficacy and complications of two groups were analyzed by application of statistical methods. Results Compared with the DHS group， the efficient of PFN group was significantly higher than that of DHS group； Operative time and blood loss of PFN group were significantly less than those of DHS group； Fracture healing time of PFN group was significantly shorter than that of DHS group； Complication rate of PFN group was significantly lower than that of DHS group， and the differences were all statistically significant （P＜0.05）. Conclusion DHS and PFN fixation of intertrochanteric fractures have good effects， but complications of PFN is less than that of DHS， so clinicians need to determine the fixation methods according to surgical indications.
