N末端B型钠尿肽原在慢性肾功能衰竭应用价值[摘要] 目的:探讨N末端B型钠尿肽原(NT-proBNP)测定在慢性肾功能衰竭疾病中的应用价值。方法:用电化学发光免疫法体外定量检测46例正常人和97例慢性肾功能衰竭患者血清NT-proBNP含量。结果:正常对照组血清NT-proBNP水平为(47.82±31.52) pg/ml;慢性肾功能衰竭患者的NT-proBNP值为(514.23±289.51) pg/ml,与血肌酐作相关性分析(r=0.380,n=97);心功能异常的肾病患者与对照组比较,差异有统计学意义(P<0.01),而无心功异常的肾病患者与对照组比较差异无统计学意义(P>0.05);血透患者达到干体重组NT-proBNP与正常对照组比较差异无统计学意义,而未达到干体重组NT-proBNP与正常对照组的结果比较差异有统计学意义。结论:慢性肾功能衰竭患者 BNP水平升高,心功能不全是主要影响因素;NT-proBNP测定在慢性肾功能衰竭疾病中具有筛查心功能不全患者、评价血透患者干体重及评价预后和疗效等临床应用价值。 [关键词] 慢性肾功能衰竭;N末端B型钠尿肽原;电化学发光免疫分析;应用价值 [中图分类号] R466.1 [文献标识码] C[文章编号] 1674-4721(2011)08(b)-096-02 Clinical value ofN-terminal pro-B type natriutetic peptide (NT-p roBNP) in chronic renal failure ZHONG Shuqi, ZHANG Jingfeng, YANG Jian Foshan Hospital of Traditional Chinese Medicine, Guangdong Province, Foshan 528200, China [Abstract] Objective: To investigate the clinical value of N-terminal pro-B type natriutetic peptide (NT-p roBNP) in chronic renal failure. Methods: The levels of serum NT-proBNP in healthy people (n=46) and chronic renal failure people(n=97) were measured by electro-chemiluminescent immunoassay. Results: The mean concentrations of NT-proBNP were (47.82±31.5) pg/ml in healthy people and (514.23±289.51) pg/ml in chronic renal failure people. Ee made correlation between NT-proBNP and Cr (r=0.380,n=97);NT-proBNP in renal patients with heart disease were significantly different with controls (P<0.01), but patients with no heart disease were not, there was no significant difference (P>0.05); Similarly, NT-proBNP in hemodialysis with lower dry weight were significantly different with controls, but higher dry weight not. Conclusion: The main reason of NT-proBNP increasing in chronic renal failure people is heart failure. So we can find heart failure, investigate the dry weight of hemodialysis and the clinical value by measuring NT-proBNP of chronic renal failure people.
