四肢软组织肉瘤临床研究【摘要】目的：探讨四肢软组织肉瘤疗效与肿瘤分期、手术方法的关系。方法：对31例不同病期四肢软组织肉瘤患者行局部广泛切除(22例)和截肢(9例)术。结果：22例局部广泛切除术中，Ⅰ期18例，其中15例术后存活5～15年无复发转移，3例局部复发;Ⅲ期、Ⅳａ期3例术后发生局部复发转移。9例截肢术中，3例术后1年6个月肺转移死亡，6例术后存活4～15年无复发转移。结论：Ⅰ期肉瘤局部广泛切除术后疗效较好，Ⅲ期、Ⅳａ期局部广泛切除术后疗效不佳，宜作一期手术截肢。手术切除应彻底，术后可辅以放疗。 【关键词】软组织肉瘤；四肢 文章编号：1009-5519（2007）22-3346-02 中图分类号：R73 文献标识码：A Clinical analysis of 31 cases of soft tissue sarcomata in extremities ZHAO Bin （Department of Surgery，Liny Industrial Trade DevelopmentHospital，Shandong 276000，China) 【Abstract】Objectice：To study the relationship of surgical trteatment， pathological stages and prognosis of soft tissue sarcoma In extremities. Methods：31 cases of soft tissue sarcoma in extremities in different pathological stages were reviewed. 22 caseswere treated with wide excision and 9 cases were treated with amputation. Results：Among 22 cases treated with wide excision，16 cases were in gradeⅠ，11 cases had survived for 5 to 15 years without relapse and 3 cases had local recurrence. In 9 cases treated with amputation，3 cases died of lung metastasis within 18 months，6 cases survived for 4 to 15 years without relapse. Conclusion：The prognosis of soft tissue sarcoma in extremities in gradeⅠwith wide excision is better than that in grade Ⅲ or Ⅳ， if the local wide excision for patients with grade Ⅲ or Ⅳ is not good，the amputation or disarticulation is recommended.The extent of local resection should be wide enough or radical，and adjuvant radiotherapy is advisable. 【Key words】Soft tissue sarcoma；Extremities 软组织肉瘤(soft tissues sarcomas，STS)是源于间叶组织和与其交织生长外胚层神经组织的恶性肿瘤，包括除淋巴造血组织外的非上皮组织，即纤维、脂肪、肌肉、间皮以及分布于这些组织中的血管、淋巴管和外周神经，多位于四肢、尤以下肢为最多。占成人恶性肿瘤的1%，儿童肿瘤的10%。软组织肉瘤因组织起源不同，症状、体征、病理分级、肿瘤分期、治疗方法及预后均不相同。1986年7月～2003年9月，我院共收治31例四肢软组织肉瘤，术后均经病理证实，随访1～15年，现将结果报道如下。 1 资料与方法 1.1 病例资料：本组31例，男16例，女15例，年龄11～70岁。其中纤维肉瘤10例，滑膜肉瘤7例，恶性纤维组织细胞瘤6例，脂肪肉瘤4例，黏液脂肪肉瘤1例，恶性间叶瘤1例，恶性神经鞘瘤1例，恶性梭形细胞瘤1例。下肢28例(92 3%)，上肢仅3例(7.7%)。肿瘤4～41 cm(5 cm 19例，5～10 cm 5例，15～41 cm 7例)。4例疼痛，3例酸痛，24例无症状。病程1年以内25例，3、5、9、14、2
