(1) A permit shall be in the form prescribed in Schedule 1.
(2) A licence shall be in the form prescribed in Schedule 2.

(1) Six weeks before each anniversary of the date of issue or renewal of a licence, the Authority may issue a
demand note and a notification to the licensee stating- (L.N. 135 of 2000)
(a) the amount of annual fee payable under section 21A(b) or 23(5A)(b) (as the case may be) of the
Ordinance and the due date therefor;
(b) that failure to pay that fee on or before that date will invalidate the licence and that the invalidated
licence will be revoked on
