布洛芬及吲哚美辛治疗婴儿动脉导管未闭对照研究的meta分析 杨群芳(重庆医科大学附属儿童医院,重庆 400014) [中图分类号]R722.1 [文献标识码]A [摘要]目的:布洛芬及吲哚美辛在临床广泛用于治疗婴儿动脉导管未闭(PDA),两种药物均有不同程度的副作用,本文通过对两种药物的随即分组治疗研究进行Meta分析,旨在探讨布洛芬及吲哚美辛治疗婴儿动脉导管未闭的疗效及并发症的差异。方法:全面检索不同数据库有关布洛芬及吲哚美辛治疗动脉导管未闭的随机对照研究,筛选出符合条件的文献,对其研究结果进行Meta分析,分析其对PDA的闭合率、坏死性小肠结肠炎(NEC)的产生、尿量情况、肾功能(Bun)的差异。结果:共纳入17篇文献。Meta分析结果显示,布洛芬与吲哚美辛对动脉导管的闭合率基本相同,无统计学差异,但布洛芬出现少尿显著少于吲哚美辛,且对患儿肾功能影响小于吲哚美辛,其他不良反应无统计学差异。结论:布洛芬及吲哚美辛治疗PDA疗效基本相同,NEC的产生无明显差异,但布洛芬对肾脏的副作用明显少于吲哚美辛,更适合用于临床治疗PDA。 [关键字] 动脉导管未闭;布洛芬;吲哚美辛;meta分析;婴儿 A meta-analysis of ibuprofen and indomethacin in the treatment of infants patent ductus arteriosus Yang Qunfang (Children’s Hospital of Chongqing Medical University, Chongqing 400014, China) Abstract: Object: Ibuprofen and indomethacin are widely used in the treatment of infantile patent ductus arteriosus (PDA), two drugs have their side effects. This study to investigate the Safety and efficacy of ibuprofen versus indomethacin in infants treated for patent ductus arteriosus. Methods : Comprehensive search different databases about randomized controlled study of ibuprofen and indomethacin treatment of patent ductus arteriosus , screening qualified literature, Meta-analysis of the research results, and to analysis the efficacy of the PDA, necrotizing enterocolitis, produce urine, renal function. Result: 17 articles were included.Meta-analysis results showed that PDA closure rate of ibuprofen versus indomethacin no significant difference, but oliguria in the ibuprofen group was significantly less than the indomethacin group and the effect on renal function in children less than indomethacin. Other side effect was no significant difference. Conclusion: Ibuprofen for PDA closure appears to be as effective as indomethacin. The NEC generated no significant difference, but the side effects of ibuprofen on the kidneys was significantly less than indomethacin,so ibuprofen is more suitable for use in the clinical treatment of PDA.
