快速定长变角式颈椎复位器的研制和临床应用 陈科,陈仲,靳安民,周初松,朱立新,舒小秋,张辉,闵少雄,张力 (南方医科大学珠江医院骨科中心,广州 510282) 摘要:[目的]:研制一种下颈椎骨折脱位即时复位的新装置,并报告其临床效果。[方法]:利用颅骨牵引弓、滑块、弹簧、棘轮、支架、导轨和底板等部件研制出一种新型颈椎牵引复位器,并临床应用于下颈椎骨折脱位33例,复位后根据患者病情择期行相应的入路手术,并随访其治疗效果。[结果]: 新型颅骨牵引复位装置可即时增重、定长牵引及角度可调。33例下颈椎脱位患者全部复位成功,无一例神经损伤加重,无牵引固定钉松动或脱出。平均牵引重量为25.6kg(10kg~60kg),平均牵引时间为23.5min(3~75min),平均牵引角度为23.5°(15~35°)。平均随访时间为37个月(20~42个月)Development and clinical application of cervical reduction apparatus with the style of rapidity,fixed-length and angular variation.//CHEN Ke, CHEN Zhong,JIN An-ming, et al.Orthopaedics center,Southern Medical University,Zhu jiang Hospital,Guangzhou 510282,China Abstract: [Objective]:To develop a new instant reduction device for lower cervical fracture-dislocation and report on its clinical effects. [Method]: Thirty three cases of lower cervical fracture-dislocation were received the treatment by a new type of cervical traction and reduction apparatus which is developed by the compositon of skull traction tong, slider, spring, ratchet wheel, bracket, rail and baseplate.After reduction then flexible and reasonable operative approaches should be adopted to treat fracture and dislocation of cervical spine according to the patients condition, and its follow-up was performed for observation of treatment effects. [Results]: This new skull traction device can be increase weight instantly, predetermined length and traction angle adjustable.33 patients were all successful reducted, no case of nerve injury was aggravated, without a traction fixed screw loosed or prolapsed was found. The average traction weight,time and angle was 25.6kg(10kg~60kg),23.5min(3~75min) and 3.5 degrees(5~35 degrees),respectively. The follow-up lasted for average 37 months(20~42 months) and all case got a satisfactory outcome of follow-up. [Conclusion]: The new traction device for lower cervical fracture-dislocation have the characteristics of simple ,practical, safe and efficient and have great clinical applications . 第一作者简介:男(1986一),
