食管腺癌论文：Ghrelin在食管腺癌、Barrett食管粘膜中的表达 【中文摘要】研究Ghrelin在食管腺癌、Barrett食管(barrett esophagus, BE)和正常食管粘膜中的表达。方法：选取30例食管腺癌患者、35例Barrett食管患者及35例健康对照,所有受试者均行胃镜检查,记录内镜下表现,在食管部位行四象限活检取材,标本经10%甲醛固定后石蜡包埋,连续切片,分别用于免疫组化及改良HE染色,采用免疫组化方法检测Ghrelin在食管粘膜组织中的表达水平。结果：Ghrelin在食管腺癌组食管粘膜中的表达水平低于健康对照组和BE组,在BE组食管粘膜中的表达高于健康对照组,差异有统计学意义(1.34±0.51 vs4.86±0.82 vs 3.54±0.79,F=27.21,P0.05)。食管腺癌组中,Ghrelin在中高分化腺癌患者食管粘膜的表达水平高于低分化腺癌,差异有统计学意义(Z=4.60,P0.05)。结论：Ghrelin是生长激素促分泌物受体(GHS-R)的内源性配体,Ghrelin生物作用极为广泛。Ghrelin与GHS-R几乎表达于人体所有的正常组织中,此外多种肿瘤组织中也已检测到Ghrelin及其受体,大量证据显示Ghrelin/GHS-R轴在细胞增殖及癌症发生中起着重要作用。目前Ghrelin分子及其受体在各种恶性肿瘤中的表达这一问题上一直存在争议,争议的焦点集中在Ghrelin在肿瘤发展方面是致癌还是预防癌症发生。Ghrelin在食管肿瘤中的研究较少。Ghrelin在Barrett食管、食管腺癌粘膜中的表达不同,提示了Ghrelin在食管癌变前和癌变后的变化。食管腺癌组织恶变过程中其Ghrelin的分泌机制发生了改变,且与食管腺癌细胞分化程度有关。 【英文摘要】To investigate the expression of ghrelin of the esophageal mucosa of patients with esophageal adenocarcinoma or Barrett esophagus(BE).Methods Thirty cases in esophageal adenocarcinoma, thirty five cases in Barrett esophagus and the control group were enrolled the test. The expression of ghrelin of all specimens were detected by immuno-histochemistry.Results The expression of ghrelin in the esophageal adenocarcinoma group was lower than that in the control group or BE group. The expression of ghrelin in BE group was higher than that in the control goup (F=27.21, P0.05). In the esophageal adenocarcinoma group, the expression of ghrelin in moderately-differentiated and well-differentiated specimens were higher than that in the poorly-differentiated (Z=4.60, P0.05)Conclusions Ghrelin is the endogenous ligand of the GHS-R. Ghrelin is widely expressed in a variety of tissues, including malignant conditions. It is indicating that the ghrelin/GHS-R axis may play an important role in cell proliferation and in carcinogenesis.
