精品论文 参考文献 动力髋螺钉、Gamma钉和防旋型股骨近端髓内钉治疗老年骨质疏松性股骨粗隆间骨折的疗效对比 （河北省高碑店市医院 骨科 河北 高碑店 074000） 【摘 要】目的：探讨并对比动力髋螺钉、Gamma钉和防旋型股骨近端髓内钉治疗老年骨质疏松性股骨粗隆间骨折患者的临床疗效。方法：选取我院2006年11月~2014年11月收治的老年骨质疏松性股骨粗隆间骨折患者228例，采用随机数字表法分为三组， DHS组患者（n=74）应用动力髋螺钉治疗，Gamma组患者（n=68）应用Gamma 钉治疗，PFNA组患者（n=86）应用PFNA治疗，对比三组患者的临床疗效。结果：PFNA组手术时间、出血量与DHS组、Gamma组相比更低，且差异显著（P＜0.05），具有统计学意义；PFNA组患者并发症总发生率与DHS组、Gamma组相比更低，且比差异显著（P＜0.05），具体统计学意义。讨论：防旋型股骨近端髓内钉治疗老年骨质疏松性股骨粗隆间骨折术中出血量较少，手术时间较短，出现并发症机率较低，因此更适用于治疗老年骨质疏松性股骨粗隆间骨折。 【关键词】老年骨质疏松性股骨粗隆间骨折；动力髋螺钉；Gamma钉；PFNA 【中图分类号】R683 【文献标识码】B 【文章编号】1003-5028（2015）7-0353-01 Efficacy comparison of Dynamic Hip Screw, Gamma nail and Anti-rotation Proximal Femoral nail in treating osteoporotic intertrochanteric fracture 【Abstract】Objective: To investigate the clinical efficacy of Dynamic Hip Screw(DHS), Gamma nail and anti-rotation proximal femoral nail (PFNA) in treating osteoporotic intertrochanteric fracture. Methods: Selected 228 patients with osteoporotic intertrochanteric fracture from November 2006 - November 2014 and randomly divided them into 3 groups. The group DHS（n=74） was given Dynamic Hip Screw（DHS）treatment, the group Gamma（n=68） was given Gamma nail treatment and the group PFNA（n=86） was given anti-rotation proximal femoral nail treatment. Observed these three groupsrsquo; clinical efficacy. Results: The pairwise comparison of these three groupsrsquo; intraoperative blood loss and operation time were significant, with statistic significance（P＜0.05）, the group PFNArsquo;s surgery time was the shortest and blood bleeding was lowest ; in the pairwise comparison of these three groupsrsquo; complication incidence, group PFNArsquo;s complication incidence was the lowest, its difference was significant and had statistic significance（P＜0.05）. Conclusion: Gamma nail and anti-rotation proximal femoral have considerable effect in treating osteoporotic intertrochanteric fracture.
