精品论文 参考文献 双胎妊娠孕周、胎位及妊娠结局与分娩方式的关系分析 杜甫蓉 　　（四川省监狱管理局中心医院 四川 成都 610400） 　　【摘要】 目的：探讨双胎妊娠并发症、孕周、胎位及妊娠结局与分娩方式的关系。方法：收集我院86例双胎妊娠临床资料进行回顾性分析。结果：在37周之前的孕产妇中，随着孕周的增加，刮宫产率增加而阴道分娩率下降，不同孕周产妇的分娩方式差异有统计学意义(Plt;0.05)；但是不同胎位的妊娠分娩方式没有统计学差异(Pgt;0.05)。分娩方式对第一胎儿Apgar评分影响不大，但是阴道分娩中第二胎儿Apgar评分le;7分比例较高，新生儿体重le;2500g者比例明显高于剖宫产组，新生儿窒息率、围生儿死亡率显著也高于剖宫产分娩组，差异均有统计学意义(Plt;0.05)。结论：孕周、新生儿体重和分娩方式的关系密切，胎位对分娩方式影响不大，但是不同分娩方式围产儿情况差异较大。 　　【关键词】 双胎妊娠；孕周；胎位；分娩方式；妊娠结局；围产儿 　　【中图分类号】R714 【文献标识码】A 【文章编号】2095-1752（2015）21-0092-03 　　Analysis of the relationship between gestational weeks, fetal position and pregnancy outcome with the mode of delivery in twin pregnancy 　　Du Furong. Administration Center Hospital,Chengdu,Sichuan 610400,China 　　【Abstract 】Objective To investigate the relationship between gestational weeks. fetus and pregnancy outcome with the mode of delivery in twin pregnancy. Methods The clinical data of 86 cases of twin pregnancy were retrospectively analyzed. Results For pregnant women before 37 gestational weeks, with the increase of gestational week the rate of cesarean section increased and vaginal delivery rate decreased, the modes of delivery for different gestational week pregnancy were statistically different (Plt;0.05), but for pregnancy with different fetal position. the delivery mode was not statistically significant (Pgt;0.05). Mode of delivery had little effect on the 1st fetal Apgar score, but the proportion of 2nd fetal with Apgar scorele;7 was higher in vaginal delivery, the proportion of neonatal birth weightle;2500g, asphyxia, neonatal perinatal mortality was also higher than that of cesarean section group (Plt; 0.05). Conclusion The relationship between gestational age, birth weight and the mode of delivery was close; but the influence of fetal position for the mode of delivery was little. Pregnancy with different mode of delivery, the outcome of perinatal was obviously different. 　　【Key word 】
