精品论文 参考文献 探讨老年慢性阻塞性肺疾病合并自发性气胸的临床疗效 黑龙江省齐齐哈尔市泰来县泰来镇卫生院 162400 【摘 要】目的 探讨老年慢性阻塞性肺疾病合并自发性气胸的诊治方法以及疗效观察。方法 此次研究的对象是选择90例慢性阻塞性肺疾病合并自发性气胸老年患者，将其临床资料进行回顾性分析，并随机分为对照组和治疗组，每组45例。对照组在常规治疗的基础上实施深静脉置管抽气疗法，治疗组在常规治疗的基础上给予胸腔负压引流疗法。对比两组患者治疗效果、疼痛程度、患者满意度及住院时间、肺复张情况。结果 治疗组患者治疗效果、患者满意度及住院时间、肺复张情况均显著优于对照组，差异均具有统计学意义（Plt;0.05）；两组患者疼痛程度比较差异无统计学意义（Pgt;0.05）。结论 胸腔负压引流法治疗老年慢性阻塞性肺疾病合并自发性气胸的临床效果显著，不会给患者带来太多的疼痛，可缩短住院时间、肺复张情况也有明显改善，得到患者的一致好评，值得推广至相关临床。 【关键词】慢性阻塞性肺疾病；自发性气胸；老年；治疗 [Abstract] Objective To investigate the diagnosis and treatment of chronic obstructive pulmonary disease complicated with spontaneous pneumothorax in elderly patients. Methods 90 cases of chronic obstructive pulmonary disease complicated with spontaneous pneumothorax in elderly patients，the clinical data were retrospectively analyzed，and randomly divided into control group and treatment group，45 cases in each group. On the basis of routine treatment，the control group was given the deep vein catheter，and the treatment group was given the negative pressure drainage therapy on the basis of conventional treatment. Compared with the two groups of patients with treatment effect，pain，patient satisfaction and hospital stay，pulmonary rehabilitation. Results in the treatment group，the treatment effect，patient satisfaction and hospital stay，lung recruitment situation were significantly better than the control group compared with statistical significance（P lt; 0.05）. The difference between the two groups of patients with pain without statistical significance（P gt; 0.05）. Conclusion negative pressure in the pleural space drainage treatment of elderly patients with chronic obstructive pulmonary disease combined with spontaneous pneumothorax in a significant clinical effect，wont bring too much pain to the patient，can shorten the hospitalization time，lung recruitment situation has improved significantly，with comments from，it is worth of clinical promotion.
