This policy provides guidelines for the staff to take the leaves they are entitled to. 两天（包括两天）以下的假期可由部门经理批准，超过两天的假期必须得到总经理的批准。 Two days ( including two days) leave should be authorized by department manager, exceed two days leave must be authorized by General Manager. 所有的请假单（病假除外）须由员工本人于休假前送往人事部门备案，病假请假单须在上班后一个工作日内送至人事部门备案，否则视为旷工。 “Request for Leave” (not including sick leave) form must be sent to HR department for record ahead of leave. “Request for Sick Leave” form must be sent to HR department for record within one day of working, or will be considered to be absent from work. 敏视达公司要求所有员工正点上班，高质量完成自己的任务。如果员工的行为影响了工作，敏视达公司将给予纪律处分，这些处分包括但不限于：警告、停职、解雇。 METSTAR requires all employees to be on time, and to complete their assigned duties with an expected level of quality. If an employee’s promptness and/or attendance interfere with those responsibilities, METSTAR will take the appropriate disciplinary action, including but not limited to: reprimand, suspension, and dismissal. 敏视达公司遵守国家法律，所有正式员工享有以下假期： METSTAR in conjunction with state laws allows absences for the following situations: 年假 Annual Leave 病假 Sick leave 产假 Maternity Leave 婚假 Marriage Leave 事假 Personal Leave 丧假 Condolence leave 敏视达公司保留要求员工提供相关证据的权利。 METSTAR reserves the right to ask for documentation in any of the above-mentioned situations. 员工应在休假前及时通知相关人员，未经请假离开公司3个工作日（包括三个工作日）以上视为自动离职，停发工资。 If an employee knows he will be absent, he should immediately notify the appropriate person(s).
