Lauren Qi, Ming Chun (Hereinafter called “ the custodian” ） The parents agree to appoint to act as above student’s guardian in undertaking the custodianship during the student studying in the Canada within the valid term of this agreement. 在本协议生效日期内，学生父母同意委托监护人负责该学生在加拿大读书期间的监管。 CUSTODIANSHIP PROGRAM 监护项目： Custodian’s responsibility: 监护人职责： The custodian offers guardianship program as below and performs the responsibility under the program.托管人提供如下学生监护服务，并根据服务内容履行监护责任。 1. Send a notarized declaration of Custodianship upon receipt of a notarized parental consent to Custodianship. 在获取经过公证的学生父母委托书后,提供公证过的监管人证明书。 2. Handle all communication with the international Education Program and/or the School District regarding academic progress, attendance, punctuality, parent-teacher meetings (academic conferences), and any disciplinary action.
