In a study by Andrews and colleagues, observers of ICU rounds and meetings found that staff reported a serious adverse event in 17% of patients. Donchin and colleagues found an average of 1.7 incidents per patient per day. Of these, 29% could potentially cause significant harm, including death. Extrapolation of these results across all U.S. ICUs (based on 55,000 patients daily and 5 million annually in U.S. ICUs) suggests that approximately 85,000 errors occur every day, of which 24,650 are potentially life-threatening.
