全身麻醉复合硬膜外阻滞在老年肝胆管取石术中应用效果 　　[摘要] 目的 探讨全身麻醉复合硬膜外阻滞在老年肝胆管取石术中的应用效果。 方法 选取本院2014年8～12月收治的84例肝胆管切开取石术老年患者作为研究对象，随机分为研究组和对照组，各42例。对照组给予单纯全身麻醉，研究组在对照组的基础上复合硬膜外阻滞麻醉，比较两组的心率（HR）、平均动脉压（MAP）、麻醉药剂量和清醒时间。 结果 研究组插管时和插管后的MAP和HR水平显著低于对照组，差异有统计学意义（P0.05）。研究组的芬太尼、异氟醚、咪达唑仑以及阿曲库铵用量显著少于对照组，差异有统计学意义（P0.05）。研究组的术后清醒时间显著短于对照组，差异有统计学意义（P0.05）。 结论 与单纯全身麻醉相比，全身麻醉复合硬膜外阻滞对生命指标的稳定效果更好，清醒速度更快，且有着麻醉药物用量少的特点。 　　[关键词] 肝胆管取石术；老年患者；全身麻醉术；硬膜外阻滞 　　[中图分类号] R614.2 [文献标识码] A [文章编号] 1674-4721（2015）08（b）-0098-03 　　Combination of general anesthesia with epidural block for Application effect of general anesthesia combined with epidural anesthesia in the elderly choledocholithotomy 　　MO Zhi-wu HUANG Zhi-dong PEI Run-ping 　　The Second People′s Hospital of Zhanjiang City in Guangdong Province；The Second Affiliated Hospital of Guangdong Medical College，Zhanjiang 514002，China 　　[Abstract] Objective To explore the application effect of general anesthesia combined with epidural anesthesia in the elderly choledocholithotomy. Methods 84 elderly patients with choledochoilthotomy from August 2014 to December in our hospital were selected and randomly divided into the control group and the experimental group，40 cases in each group.The control group was given general anesthesia，the experimental group was given epidural block on the basis of the control group.The level of HR，MAP，anesthetic dose and awake time in two groups was compared. Results The level of MAP and HR during intubation and after intubation in the experimental group was lower than that in the control group，with significant difference（P0.05）.The dose of isoflurane，fentanyl，midazolam and atracurium in the experimental group was less than that in the control group，with significant difference（P0.05）.The awake time at the end of operation in the experimental group was shorter than that in the control group，with significant difference（P0.05）.
