PAGE II 摘 要 本次研究以探讨大学生健康评估系统为目的，以在校大学生为研究对象，为分析大学生健康状态及即时作出科学的评估提供了科学依据和实践工具。 该系统与国内外标准健康量表紧密结合，利用所研究出的大学生健康评估表，成功生成健康评估系统。该系统将对大学生对自己的健康认知起到有力的推动作用。 本文针对该评估系统的研究和设计展开，首先讲述课题的重要意义，介绍数据库的基本知识、健康管理的概念和重要意义、数据库建立的原则和方法。之后对系统进行需求分析。在数据库的设计上，采用了Access 2013软件，首先根据需求分析建立了模块，再建立了表，确定了各表之间的联系，最后进行了登陆模块、基本信息模块、问卷模块和评估模块的窗体设计。最后，本文给出了四个主要模块的运行效果图，指出了本论文的主要工作和系统的一些特征，说明了本系统的不足以及可能作出的改进。 当今，通过采取科学、有效的大学生健康评估模式，能够避免传统模式中不病不治、不治不防的静态管理缺陷；配合健康评估系统的应用，将大学生健康信息电子化、健康评估自动化，使健康评估的平台具有了开放性、互动性和即时性。为统计分析健康信息、收集整理评估结果、提高学校卫生部门工作效率提供了有利工具。而且，健康评估系统在提升大学生的健康意识、健康状态和参与积极性的同时，起到了预防疾病、控制危险因素的目的，减轻了社会、高校和学生家庭的医疗负担，为培养健康合格的高素质人才奠定了基础。 关键词：大学生；健康评估；Access Abstract For the purpose of this study in order to investigate college students health assessment system, to college students as the research object, for the analysis of college students health status and instant to make scientific assessment provides scientific basis and practical tools. The system is closely integrated with the domestic and international standard health scale, using the study of college students health assessment, success generates health assessment system. The system will be on college students cognition to the health of their play a powerful role in promoting. Based on the evaluation system of research and design, first about the topics of significance, introduce the basic knowledge of database, the concept of health management and the significance, principle and methods of the database established. After the system requirements analysis, choose the B/S model of development. On the design of the database, using the Access 2013 software, according to the requirement analysis module is established first, then set up a table, confirmed the link between each table, finally has carried on the landing module, basic information module, the questionnaire form design module and evaluation module.
