The subsequent ATP II report, released in 1993, incorporated findings from angiographic trials and large meta-analyses in a format that provided clinicians with recommendations for clinical practice. ATP II recommended that all patients with established coronary heart disease (CHD) be treated to a goal low-density lipoprotein cholesterol (LDL-C) of ?100 mg/dl. Since 1993, substantial scientific evidence has been gathered, particularly with regard to the benefits of LDL-C reduction with HMG-CoA reductase inhibitors (statins). Large trials have linked the use of some of these agents with the decreased incidence of major coronary events and mortality in patients regardless of age or gender, and benefits are not limited to those with high cholesterol levels and CHD but appear to extend to patients treated for primary prevention and average cholesterol levels as well. The new NCEP ATP-III guidelines released in 2001 extend many of the principles of the ATP II guidelines and created a new category of patients with CHD risk equivalents. The guidelines were created with the expectation that physicians would integrate these recommendations into their routine clinical practice and that as a result there would be substantial improvements in cardiovascular health.
