potentialrisks,andtheexpectedbenefits.
3.FinancialResponsibility.
TheOwneragreestobefinanciallyresponsibleforall
costsassociatedwiththesurgeryandanyadditional
treatmentrequiredduetocomplicationsarisingfromthe
surgery.TheOwnerunderstandsthatanestimateofthe
costswillbeprovided,butthefinalamountmayvary
dependingontheactualservicesrendered.
4.Post-SurgicalCare.
TheOwneracknowledgesthattheyareresponsiblefor
providingappropriatepost-surgicalcareasinstructedby
theHospital.TheOwnerunderstandsthatfailuretofollow
thepost-surgicalcareinstructionsmayresultin
complicationsandadditionalmedicalexpenses.
5.ReleaseandIndemnification.
TheOwnerreleasestheHospital,itsstaff,and
veterinariansfromanyliabilityorclaimsarisingfromthe
surgery,exceptforgrossnegligenceorwillfulmisconduct.
