导管相关性血流感染患者原因分析与预防控制

目的探讨导管相关性血流感染的原因及对策。方法选择2007年3月～

2010年3月共450例多途径中心静脉置管病例，其中，发生导管相关性血流感

染者10例。10例分别从留置导管中和其他外周血管采血并且剪下导管尖端培养，

均培养出同种细菌，1例培养出白色念珠菌。结果本组病例从发现后24h内即

明确诊断导管相关性血流感染的患者8例（占80%），就诊后2～4d内明确诊断

者2例（占20%）。其中内科感染例数为7例，感染率为1.75%；外科感染例数

为2例，感染率为6.67%；其他感染例数为1例，感染率为5%。结论应提前预

防控制导管相关性血流感染，从而缩短住院时间，节约医疗资源，减轻患者痛苦。

【Abstract】ObjectiveToexploreanddiscussthecauseofthecatheter-related

bloodstreaminfectionanditscountermeasures.MethodsSelect450casesof

multi-channelcentralvenouscatheterthatweretreatedfromMarch2007tillMarch

2010，amongwhichtencaseswerethoseofcatheter-relatedbloodstreaminfection.

Forthe10cases，bloodwasrespectivelycollectedfromtheoriginalcatheteraswellas

fromtheotherperipheralcatheter，andcutthecathetertipforcultivation.Asaresult，

thesamespeciesofbacteriawerecultivated，withonecaseresultinginthewhite

candidaalbicanscultivated.Results8caseswerediagnosedascatheter-relatedblood

streaminfectionwithin24hours（covering80%）andtwopatientsweredefinitely

treatedwithin2-4daysafterthediagnosis（covering20%）.Amongtheseten

patients，sevenwerecasesofinfection，theinfectionratebeing1.75%;twopatients

werecasesofsurgeryinfection，theinfectionratebeing6.67%;andonlyonepatient

wasdiagnosedasotherkindofinfection，theinfectionratebeing5%.Conclusion

Preventionandcontrolshouldbemadeaheadofcatheter-relatedbloodstream

infection，inordertoshortenthelengthofhospitalstay，tosavehealthcareresources

andtoreducepatientssuffering.
