[Abstract]ObjectiveToexploretherelationshipbetweentheseverityofwhitematterhyperintensities（WMH）andthedistributionofcollateraldiseasesyndrometypes.MethodsAtotalof208patientsclearlydiagnosedwithcerebralsmallvesseldiseaseinLishuiTCMHospitalAffiliatedtoZhejiangChineseMedicalUniversityfromDecember2022toDecember2023wereselectedasthestudysubjects.Allenrolledsubjectsinthestudycompletedthecollateraldiseasesyndromedifferentiation.WMHwasgradedonmagneticresonancenbsp;imagingfluid-attenuatedinversionrecovery（FLAIR）sequencesusingtheFazekasscaleandmeasuredforvolume.ResultsAmongthe208patients，therewere52casesofthebraincollateralimpoverishment（BCI）type，withFazekasscoreof3（2，3）points，andWMHvolumeof（8.80±9.37）ml;67casesofthebraincollateralstasis（BCS）type，withFazekasscoreof4（3，4）points，andWMHvolumeof（13.42±9.18）ml;47casesofthebraincollateralconstraint（BCC）type，withFazekasscoreof4（3，5）points，andWMHvolumeof（14.60±10.07）ml;and42casesofthebraincollateralocclusion（BCO）type，withFazekasscoreof4（4，5）points，andWMHvolumeof（18.65±10.71）ml.Ofthese，55patientswithaFazekasscoreof1-2pointsweremostcommonlyoftheBCItype，105patientswithaFazekasscoreof3-4pointsweremostcommonlyoftheBCStype，and48patientswithaFazekasscoreof5-6pointsweremostcommonlyoftheBCOtype.TherewerestatisticallysignificantdifferencesinFazekasscoresandWMHvolumesbetweentheBCIandBCOtypesandothersyndrometypes（Plt;0.05）.ConclusionTheseverityofWMHcorrespondstoacertaindistributio
